How To See Dependents' Profiles
- Login to my.levelhealthplans.com
- Click on the Family tab
- Preview your family's member ID cards
- If dependent is under 13 years old, you may see their full profile
- If dependent is over 13 years old, you will need a PHI release form to continue viewing the profile.
How to find a PHI Release Form:
- On the dependent's profile, they may click the tab "Forms" on the left of the portal
- Click "PHI Release"
- Dependents can assign a guardian as an authorized individual on their account and press "Submit"
